| 1 to 39 | Critical | Evidence indicates a material health concern; complete same-shift diagnostic review. |
| Pending | No valid record | Health is withheld until a timestamped governed health record exists. |

The Optimal® Trend health score is a governed 0 to 100 record produced from live measurement evidence inside the configured evidence window. Scores update when the backend producer writes a new timestamped health record; the page displays that record and does not calculate the score.
Measurement threshold state (40%) – highest measurement threshold severity observed inside the evidence window.
Data freshness (25%) – most recent live measurement timestamp inside the evidence window.
Sensor coverage (20%) – mapped measurement points with evidence inside the evidence window.
Active alert burden (15%) – active alert count and highest alert severity inside the evidence window.
